Make a Referral

Anyone is welcome to refer a child for services. These people may include parents, relatives, doctors & health care professionals, teachers, etc. The parent of the child must be aware and give their consent for the referral.

To make a referral, please complete one of the agencies referral forms. You can do this by clicking on one of the links below, or by phoning the centre. We would be happy to email or fax you a form if required.

The Children’s Therapy & Family Resource Centre provides two different referral forms to be used appropriately, based on the age of the child.
